What is Rapid Response?
As part of our dedicated Rapid Response Team, you will be helping patients leave the hospital as soon as they no longer need medical care, without waiting for extra assessments. Instead of staying in a hospital bed, patients can return home or to a community setting, where our team will assess their needs in a more comfortable environment.
This approach frees up hospital beds for those who need urgent care, while allowing patients to recover at home. After discharge, a personalised care plan is created to support their recovery. We work closely with family and other medical professionals to ensure the best quality of care going forward.
